Forest Trail New Student Enrollment Forms for
2006/2007
Dear
Parent,
Thank
you for selecting Forest Trail Elementary as your child’s
school. As a staff, we recognize and appreciate the trust
that you place in us when you allow your “most prized
possession” to join us. We
take the
roll
as educators
of
young children very seriously and
solicit your participation and cooperation in creating the
best possible learning situation for your child.
The
forms that follow are required to register your child at our
school. You may print copies and bring the completed
information with you when you come to officially register in
person. This should expedite the process and allow you to
complete most of the paperwork in the comfort of your own
home or office.
In
addition to these forms, please bring the following items
with you to register:

Proof of residency – phone bill,
lease agreement, purchase agreement, etc.

A copy of the Social Security
Card.

Birth Certificate – hospital
record is not accepted
-
Immunization record signed by a
physician.

Valid Drivers License
During
the school year our office hours are from 7:30 A.M. – 4:00
P.M. During the summer our school offices are open 7:00 A.M.
– 3:00 P.M. through June 16th and will reopen for the fall
term starting on July 20th. Please call 512-732-9160 to
schedule a registration appointment.
